Madden NFL 12 jest w dalszym ciągu realistycznym i jednocześnie widowiskowym symulatorem amerykańskiego futbolu. Gracze mogą wcielić się w zawodników wszystkich 32 drużyn i zagrać na dobrze znanych stadionach. Gra pozwala zmierzyć się w prawdziwych zawodach (Super Bowl) oraz stworzyć zespół marzeń (w trybie Madden Ultimate Team).

Twórcy gry przygotowali podobny zestaw funkcji i trybów rozgrywki jak przed rokiem, chociaż wzbogacili go o kilka dodatków. Podczas rozgrywek oglądamy nowe telewizyjne wstawki, w których pojawią się maskotki drużyn. Producent podrasował wygląd stadionów, oświetlenie i pracę kamer, a także wprowadził trójwymiarową trawę. Nie mogło zabraknąć zmian w rozgrywce – dotyczą one głównie umiejętności zawodników na boisku.
